By signing up for and/or attending classes, events, activities, and other programs (including any on-line or virtual classes, events, activities, and other programs) and/or using the premises, facilities and/or equipment (individually and/or collectively, the “Classes and Facilities”) of Bunk 5, LLC dba Ripped Fitness, and its subsidiaries and affiliates, including Ripped Live, LLC (collectively, “Ripped Fitness”), I acknowledge that there are certain risks inherent to the Classes and Facilities at Ripped Fitness that cannot be eliminated due in part to their arduous nature and regardless of the care taken to avoid injuries. These risks include, but are not limited to (1) minor injuries such as scratches, bruises, and sprains; (2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, abnormal blood pressure, fainting, and concussions; and (3) catastrophic injuries, including paralysis and death.  

Because physical exercise can be strenuous and subject to risk of injury, including serious injury or death, I confirm that Ripped Fitness has urged me to obtain a physical examination from a doctor before engaging in the Classes and Facilities.  I also confirm that I have voluntarily chosen to participate in the Classes and Facilities and I agree that at all times I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions given to me by Ripped Fitness staff. If in the subjective opinion of the Ripped Fitness staff, I would be at physical risk participating in Classes of Ripped Fitness, I understand and agree that I may be denied access to the Classes and Facilities until I furnish Ripped Fitness with an opinion letter from my medical doctor, at my sole cost and expense, specifically addressing the concerns of Ripped Fitness and stating that these concerns are unfounded. 

I agree that if I engage in the Classes and Facilities, I do so entirely at my own risk. In addition, I acknowledge that dietary modifications made in connection with the Classes and Facilities can cause a variety of changes and can create problems, including as it relates to my energy, stamina, appetite, and mood. Any recommendation for changes made by Ripped Fitness or anyone associated with Ripped Fitness regarding diet, including the use of food supplements, weight reduction and/or body building enhancement products are entirely my responsibility and I have been advised to consult a physician prior to undergoing any dietary or food supplement changes.
